The City Concern Form provides residents with an easy way to share questions, concerns, or requests with City staff. Whether it’s reporting a maintenance issue or raising a neighborhood concern, this form helps us track and respond more efficiently.

Please include as much detail as possible, such as the location (address or landmark) and a brief description of the concern. Once submitted, your form will be directed to the appropriate department for follow-up.

All information provided on this form will remain confidential and will only be used by City Staff to address concerns.
